Myeloperoxidase (MPO) is a tetrameric heme-containing protein abundantly produced in neutrophil granulocytes, where it plays a critical anti-microbial role. Stored in azurophilic granules, MPO is released during neutrophil degranulation and contributes to the formation of hypochlorous acid from hydrogen peroxide and chloride ions in the ‘respiratory burst.’ It also catalyzes the oxidation of tyrosine to tyrosyl radicals, both of which are cytotoxic agents against bacteria and other pathogens. A hereditary MPO deficiency can lead to increased susceptibility to infections.
MPO levels have significant clinical implications, especially in cardiology. Numerous studies have established a correlation between elevated MPO levels and coronary artery diseases, positioning MPO as a potential predictor for myocardial infarction in patients with chest pain.
